System Director, Hospital Care Management

About the role

HealthPartners is currently hiring for a System Director, Hospital Care Management. This position is responsible for directing HealthPartners’ Hospital Care Management service line which encompasses clinical and operational case management, social work, program management, and data/reporting analytics across all hospitals within the care family. 

This key leadership position reports to the Vice President of Home & Community Services and Value Care Strategies and has a strong partnership and relationship with Hospital Vice Presidents of Patient Care/CNOs. 

The Director must be an involved leader, who can build trust and influence a wide variety of stakeholder groups. Critical to this role’s success will be this leader’s ability to build relationships across all levels of individual contributors and leadership. This is a new opportunity available within the HealthPartners care family with a team of three (3) direct reports and total accountability of seventy plus (70+) staff members.

 

Required Qualifications:

  • Bachelor’s degree in healthcare related discipline. 
  • Seven (7) years of hospital or healthcare related leadership; with progressive and increased relevant leadership experience in care management. 
  • Valid and current discipline license in state of Minnesota and Wisconsin as required for individual’s discipline.
  • Demonstrated experience to partner with colleagues to lead programs and initiatives successfully across multiple settings of care.
  • Knowledge of principles and practices of healthcare administration and healthcare delivery systems
  • Demonstrated ability to build and execute a strategy for multi hospital system programs and services.
  • Skilled in making decisions under challenging situations.
  • Demonstrated skill in effective use and management of automated medical management systems, electronic medical record and additional technology, software, and tools to support the work.
  • Demonstrated ability to lead multi-site leaders effectively.
  • Demonstrated knowledge of and experience with budgetary responsibilities
  • General business administration skills
  • Ability to communicate and present effectively to large and small audiences, including executive stakeholders and leaders.
  • Ability to identify, understand and manage project and business risk.
  • Demonstrated capability to manage multiple projects and activities.
  • Demonstrated experience in hiring, orienting, coaching, and team building of professional staff.
  • Excellent interpersonal communication skills, group process, meeting management, conflict resolutions skills.
  • Knowledge of applicable external organization regulations/laws pertaining to hospital care management and post-acute care.
  • Strong analytical skills required.
  • Experience and knowledge of value-based care and accountable care organization/quadruple aim measures including appropriate setting of care utilization, alternatives to admissions, readmission prevention, progression of care and length of stay across the continuum.
  • Skilled in the complexities of transitions in care and experience designing and driving effective post-acute care strategies.
  • Experience working across a healthcare system in support of overall care delivery initiatives and organizational goals by designing, implementing, and measuring specific care models to approach the work and achieve the goals.
  • Experience with hospital flow principles and the development of progression of care/length of stay and readmission prevention strategies.
  • Leadership experience with demonstrated success in managing various clinical disciplines:  physicians, nurses, social workers, and other clinicians.
  • Experience interacting with and partnering with senior executives and physician leaders internally, across the organization and externally.
  • Understanding of the external environment including CMS, State and County government influencing healthcare and initiatives
  • Excellent interpersonal communication skills, group process, meeting management, conflict resolutions skills.

Preferred Qualifications:

  • Master’s degree in social work or bachelor’s in nursing or other related healthcare discipline. 
  • Ten plus (10+) years of hospital or healthcare related leadership; with progressive and increased relevant leadership in care management. 
  • Valid and current Minnesota licensure as an RN or Social Worker (LICSW).
  • Project management and business planning experience; in hospital or healthcare setting.
  • Experience working across departments and organizations strongly preferred. 
  • Experience successfully implementing programs or services for multi-hospital system
