Description
The Adult Case Manager provides community-based case management and care coordination services to adults experiencing serious mental illness, co-occurring disorders, and complex psychosocial challenges. This position supports individuals in achieving stability, independence, recovery, and improved quality of life through person-centered, recovery-oriented, and trauma-informed practices.
The Adult Case Manager assists clients in accessing medical, behavioral health, housing, financial, vocational, educational, and community resources while coordinating care across multiple providers and systems. The role works collaboratively within an interdisciplinary treatment team to support continuity of care, reduce barriers to treatment engagement, promote wellness and recovery, and minimize unnecessary hospitalization or crisis service utilization.
This position requires strong organizational skills, professional judgment, advocacy abilities, and the capacity to build therapeutic and supportive relationships while maintaining professional boundaries. Services are provided in office, community, residential, hospital, and home-based settings as clinically appropriate.

Requirements
MINIMUM QUALIFICATIONS
Bachelor’s degree in social work, psychology, counseling, rehabilitation, human services, or a related field; OR
Associate degree in a human services-related field with relevant experience in behavioral health or community-based services.
Experience working with adults with serious mental illness, co-occurring disorders, or complex psychosocial needs preferred.
Demonstrated ability to work collaboratively within a multidisciplinary team.
Strong communication, organizational, and interpersonal skills.
Ability to work independently and manage community-based responsibilities.
Experience with electronic medical record systems preferred.
Preferred Qualifications
Experience in community mental health, targeted case management, residential services, or psychosocial rehabilitation programs.
Familiarity with Medicaid documentation and Targeted Case Management requirements.
Experience coordinating services with hospitals, housing agencies, or state/community systems.
Knowledge of recovery-oriented and trauma-informed practices.
Experience supporting individuals with severe and persistent mental illness.
OTHER EXPECTATIONS
Maintain valid driver’s license, acceptable driving record, proof of automobile insurance, and reliable transportation.
Ability to transport clients when clinically appropriate and authorized by agency policy.
Ability to travel throughout the agency service area as required.
OSHA RATING
This position is classified as Medium Risk due to regular direct client contact, potential behavioral health crises, community-based work, and possible exposure to bloodborne pathogens. The employee is expected to comply with all OSHA regulations and agency safety policies, including infection control procedures, universal precautions, and required safety trainings. Hepatitis vaccinations are available.
