Schedule: Monday - Friday, 8:00AM - 4:30PM

 

SUMMARY:

Responsible for coordination of activities such as data collection, analysis and education that support the inpatient rehabilitation process including the Prospective Payment System (PPS) and supporting the interdisciplinary case management processes of the inpatient rehabilitation unit.

 

RESPONSIBILITIES:

•  IRF-PAI Coordination & Data Management: Completes and submits IRF-PAI using clinical data and Care Connect documentation; ensures timely and accurate demographic, clinical, and Quality Indicator (QI) data entry; communicates estimated length of stay for effective discharge planning; tracks 60% rule compliance.
• Interdisciplinary Collaboration & Education: Works closely with care teams to ensure accurate diagnosis coding, plan of care development, and documentation; educates staff on medical necessity, QI scoring, and regulatory requirements; conducts competency checks and supports re-credentialing processes.
• Quality Improvement Participation: Supports regulatory compliance (DNV, CARF, etc.) by identifying performance improvement opportunities; helps develop and implement quality action plans and rehab policies; performs data analysis and outcome reporting to enhance care quality.
• Data Analysis & Reporting: Regularly compiles and analyzes outcome data; creates charts, graphs, and reports using Excel; supports patient experience initiatives through follow-up data collection and participates in quality improvement meetings.

REQUIRED QUALIFICATIONS:

• Required to have one of the following: Bachelor's degree in Nursing, Occupational Therapy, Therapeutic Recreation, Physical Therapy; (for degree prior to 2016) Master's degree in Speech Language Pathology OR (for degree after 2016)Doctor of Physical Therapy
• Minimum of three years of experience in a clinical setting
• Current NYS licensed Registered Nurse, Occupational Therapist, Therapeutic Recreation Specialist, Speech Language Pathologist or Physical Therapist

PREFERRED QUALIFICATIONS:

• Three years of clinical experience in a rehabilitation setting.
• Working knowledge of the Medicare Prospective Payment System.
• Knowledge of ICD-10 coding, CMG classification and IRF-PAI.
• Basic computer skills including: Word processing, Excel spreadsheets and familiarity with web-based software applications.
• Current NYS license in Social Work, Physical Therapy, Occupational Therapy or Speech Language Pathology

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
