LV HOME CARE AND HOSPICE
REGISTERED NURSE (RN) - HOSPICE INTAKE/TRIAGE NURSE - WEEKENDS
Summary
Coordinates referrals and physician orders for patients discharging to Hospice services. Facilitates communication between staff, referral sources, insurance companies, and physicians. Performs phone triage, data/order entry, patient/staff scheduling, and other clinical tasks related to the provision of patient care. Provides clinical support to the hospice team by assisting in the coordination of patient care services.
Job Duties
- Collects data and evaluates patient appropriateness for Hospice Services through prescreening and communication with healthcare team members, and facilitates patient transition to appropriate level of hospice care.
- Triages patient/caregiver phone calls utilizing critical thinking skills and sound clinical judgement.
- Performs data/order entry, patient/staff scheduling, and other clinical tasks related to the provision of patient care.
- Provides hand-off communication to members of the care team as appropriate regarding triage information, patient status updates, and patients who require a follow up visit or a phone call.
Minimum Qualifications
- Specialized Diploma NLN approved/accredited nursing program
- 1 year nursing experience in a healthcare setting and
- 1 year direct nursing care experience
- Demonstrates knowledge and ability to care for patients based on the population served
- Demonstrates critical thinking skills and sound clinical judgement
- Demonstrates analytical and problem-solving skills
- Ability to perform duties with accuracy and attention to detail
- Demonstrates excellent verbal and written communication skills
- Demonstrates excellent customer service skills
- Demonstrates team behaviors and the ability to work independently
- Demonstrates proficiency in basic computer skills
- Demonstrates excellent time management, organization, and prioritization skills to manage a heavy workload
- Ability to multitask and work with multiple interruptions
- Ability to adapt to changing priorities
- Demonstrates evidence of competency as per department/unit specific competency assessment criteria
- Knowledge of agency and network clinical policies, procedures, and resource materials
- BLS - Basic Life Support AHA - American Heart Association Within 30 Days and
- RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire and
- DL - Driver's License_PA - State of Pennsylvania Upon Hire or
- DL - Driver's License_NJ - State of New Jersey Upon Hire
Preferred Qualifications
- Associate’s Degree Nursing or
- Bachelor’s Degree Nursing
- 1 year home health/hospice experience or
- Less than 1 year nursing experience in a clinical office setting or
- Less than 1 year nursing experience performing phone triage
- CHPN - Certified Hospice and Palliative Nurse - Hospice and Palliative Nurses Association within 3 Years
Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
