Primary Purpose of Position

The LVN/LPN provides clinically competent delivery of quality medical services required during operations within their scope of practice. The LVN/LPN working at the site is responsible for safe patient care and ensuring adherence to all site-specific Policies and Procedures.

Core Duties:

• Maintain qualifications and skills of a Licensed Practical Nurse as confirmed by completion of the Skills Checklist and verified by the clinical manager. Successfully complete the unit-specific orientation and competency skills checklist within the first 3 shifts. Maintain competency verification programs in accordance with unit, JC guidelines.

• Coordinate patient care through a continuum and facilitate the achievement of optimal outcomes in relation to care, quality, and cost-effectiveness.

• Use the nursing process as a basis for professional practice. Demonstrate proper assessment, preparation of patients for treatments and procedures, including surgical interventions.

• Perform/assist with clinic-specific procedures, in accordance with established policies and guidelines used in the MTF and IAW JC guidelines. Supervise all paraprofessionals necessary in the care and treatment of the patient.

• Skill in setting up, operating, and monitoring specialized medical equipment such as IV infusion pumps, cardiac monitoring devices, EKG units, oxygen analyzers, nebulizers, and other clinic-specific equipment.

• Excellent oral communication skills for patient/family education. Communicate treatment plans in accordance with established clinic protocols.

• Demonstrate excellent written communication skills to perform accurate documentation, both written and electronic, of all activity in accordance with requirements.

• Assist with diagnostic exams and procedures, administer medications and immunizations.

• Initiate and maintain pertinent patient and family medical history on patients, recording observations and stated symptoms for use by the Medical Home Port (MHP) Team. Observe and record pertinent vital signs, determine and order laboratory procedures on patients that may be ordered by the provider to diagnose and treat the patient, per command and department policies. Orient new patients and their families to clinic policies as required.

• Prepare equipment and assist health care providers with treatment procedures.

• Assist with clinic check-out and follow-up visits by reviewing post-visit needs with the patient.

• Ensure scheduling of follow-up appointments or specialty care referrals. Assist patients by directing or escorting to other clinical services (laboratory, radiology, pharmacy, immunizations, etc.), as required.

• When assigned, serve and participate in other functions and meetings. Provide relevant and timely information and assist with decision-making and process improvement.
