Home-Based Acute Care RN

The Home Based Acute Care Registered Nurse (RN) provides professional, comprehensive nursing care for acute care patients in the patient's home environment in collaboration with the HBAC Command Center. Accountable for the delivery of coordinated, safe, compassionate, therapeutic, evidence-based quality care to patients and families, based on individual physical, emotional, and spiritual needs, and appropriate care strategies throughout the lifespan. Practices in accordance with the Nurse Practice Act in the state of employment, the American Nurses Association (ANA) Code of Ethics for Nurses, and the ANA scope and standards of practice. Key functions and specific skills for the job are found in the unit specific onboarding checklist and in standards that are applicable to the specialty.

Required Qualifications: 

• Education sufficient for licensure.
• Washington Registered Nurse License upon hire. Or
• Oregon Registered Nurse License upon hire.
• National Provider BLS - American Heart Association upon hire.
• In compliance with the Providence Health & Services, criminal background check is required for this position upon hire, based on assigned duties.
• Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
• 1 year of Medical/surgical experience in an acute care setting.

Preferred Qualifications:

• Bachelor's Degree in Nursing
• IV/Infusion skills
• Recent Home Health experience
• Point of Care experience

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
