DEPARTMENT SUMMARY: This department is based on the St. Charles Bend campus and includes several teams that are located together in a workspace that facilitates cross-team support and collaboration system-wide. These teams include the Transfer Center, Central Communications, Staffing/Scheduling Office, Patient Logistics, and Patient Safety Attendants. Additionally, this department facilitates several critical operations, communication, and facilities functions related to security, safety, and disaster preparedness. The department is a system-wide service that ensures safe, efficient, and patient-centered care delivery across St. Charles Health System. It works to ensures that patients receive the right care, in the right place, at the right time by orchestrating patient flow, optimizing bed placement and throughput. The department leads efforts to reduce delays, eliminate barriers, and improve coordination between hospitals, clinics, and community partners.
POSITION OVERVIEW: The Nurse Manager of Patient Flow is responsible for the management of the Transfer and Communications Center that coordinates the logistics of patient placements and throughput. Additionally, this role manages centralized scheduling and staffing as well as logistics and patient safety attendants. The position requires strong skills in leading a multidisciplinary team. This role serves as the operational leader for patient movement between the St. Charles Health System sites and other community and regional facilities. They also support the Director of Care Coordination and Patient Flow in the management and support of patient movement between departments and across hospital campuses. This position directly manages supervisors of transfer center and scheduling and staffing as well as caregivers in the Transfer Center, Central Communications, and Patient Logistics.
ESSENTIAL FUNCTIONS AND DUTIES:
Responsible and accountable for successful 24/7 operation of assigned departments.
Responsible for resource management ensuring appropriate and sufficient equipment and supplies are available. Manages staffing and scheduling. Provides productivity oversight and review. Works with leadership to achieve productivity standards. Develops and implements annual plan including goals for unit with director and supervisor.
Serves as a real-time resource and assists with clinical expertise for staff with problem-solving on patient care, processes, and family issues. Serves as representative for all customers.
Assists with quality, safety, customer service and regulatory compliance oversight: SAS – Receipt, assignment, tracking and trending, delegation.
Coordinates quality initiatives and process improvements. Aligns and manages to St. Charles Health System metrics and quality initiatives.
Ensures team members’ competency in safe communication practice with HAA (Hospital Air Ambulance) agencies, EMTALA, and Joint Commission regulations related to air medical ambulance transport as well as state, county, and local regulations related to ground ambulance and inter-facility transportation and transfer of patients.
Ensures team members’ competency in implementation of labor contracts and understanding of staffing laws and regulations
Maintains ongoing communication with staff, department leadership, and others to ensure needs are being met for staff and patients. Engages medical staff to obtain input on the operation of the department and key decisions.
Supports the Director of Care Coordinator and Patient Flow in developing and maintaining physician and peer partnerships across the heal
