Full-time RN position to provide individualized nursing care at the place of residence/home of clients with acute, chronic and complex conditions.

Job Summary:

MercyOne/Trinity Health Home Care in Mason City is seeking a compassionate and dedicated Home Care Registered Nurse (RN) to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the support needed to maintain their health and independence. This position offers a $10,000 sign-on bonus.​

Schedule:

• Full-time, Monday through Friday, 8:00 AM – 4:30 PM

• Two holidays per year

• Weekend rotation based on staffing levels

• On-call only during scheduled weekends, Saturday and Sunday, 8:00 AM – 4:30 PM

• No weekday or weekend evening calls​

Position Purpose:

Provide skilled nursing care to home-based patients as prescribed by physicians and in accordance with organizational policies. Responsibilities include initial assessments, care plan development in collaboration with the Population Health Case Manager, supervision of Home Health Aides and LPNs, and ensuring optimal patient outcomes and satisfaction.​

Key Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans

• Deliver therapeutic interventions and adjust care plans as necessary

• Collaborate with physicians, case managers, and interdisciplinary teams

• Supervise and support LPNs and Home Health Aides

• Utilize motivational interviewing and health coaching techniques

• Accurately document patient interactions and care provided

• Ensure compliance with all regulatory and organizational standards

Minimum Qualifications:

• Graduate of an accredited Nursing program (BSN preferred)

• Current RN license in the state of practice

• Minimum of one year of professional nursing experience (Home Health experience preferred)

• Strong computer and technology skills

• Valid driver's license and reliable transportation​
