Job Description:

Phoenix Home Care & Hospice is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide 1:1 skilled nursing care in the home of a client in Pineville, MO. We are specifically looking for a full-time day shift nurse who is passionate about delivering personalized care in a supportive and flexible environment.

Job Responsibilities:

• Follow an individualized plan of care developed with the physician
• Monitor, document, and report the patient’s condition using electronic charting
• Educate and provide support to the client and their family
• Collaborate with the care team and attend meetings as required

Qualifications:

• Active State of Missouri or Compact Nursing License (LPN or RN)
• Strong communication and critical thinking skills
• Professional, compassionate, and positive attitude
• Ability to work independently and manage time effectively
