About this role:
Lifeworks Self-Directed Services is seeking a full-time exempt Service Coordinator to provide case management for Lifeworks Financial Management Services clients and families. As an approved Financial Management Service through the State of Minnesota, Lifeworks helps individuals enrolled in the Community First Services and Supports (CFSS) Program. This is a great position for a case manager seeking to empower people to use services in their own communities. The position works as part of a greater Self-Directed Services team and also has a lot of autonomy when managing their caseload. A successful candidate will enjoy meeting new people; have excellent interpersonal skills; deliver detailed work, stay organized and meet deadlines; and have a strong desire to successfully deliver services within the required guidelines of state statutes.
Lifeworks is also an experienced Consumer Directed Community Supports (CDCS), Consumer Support Grant (CSG), Consultation Services Provider, PCA Choice provider agency and a licensed provider of Individualized Home Support, Night Supervision, Respite, and Homemaker services.

Department: Self-Directed and In-Home Services
Hours: Full-time 40hours/week, Monday – Friday (Exempt)
Location: The role is hybrid - 20% in-office based in our Richfield, MN headquarters.
Compensation: $52,000-$60,000/year
