Position Overview:

Sutter Care at Home(SCAH) currently has a unique opportunity for an experienced Hospice Physician to serve in an administrative leadership role with medical oversight of all hospice physicians/nurse practitioners at all 7 Medicare licensed hospice agencies, plus 2 branches currently operated throughout northern California.

This role will work in a dyad relationship with the Hospice Director of Operations and report to the Chief Medical Officer of Sutter Care at Home. It is a hybrid role with frequent leadership meetings in Fairfield, and expected onsite visits to 9 hospice locations, as well as 15 contracted Sutter Health hospitals.

Directly manages all facets of Hospice Medical Directors and Physicians at Sutter Care at Home- including and not limited to their recruitment, onboarding, training, performance, productivity, documentation, quality outcomes, as well as any/all issues related to regulation/compliance.

Directly oversees all activities/performance/duties of all Nurse practitioners at SCAH, through the direct reporting of an NP Manager.

Works closely with Director of Hospice Operations in a dyad leadership model to oversee all hospice administrators to ensure alignment with growth, quality and operational strategies and goals. This will also include survey readiness and participate as needed in any hospice audits, ADRs, and/or targeted medical reviews.

Work closely with VP of Strategy/Business relations to provide support of any and all community education, growth and referral sources.

Work closely with CMO to deliver high quality standardized Hospice GIP care at all contracted facilities.

Work closely with CMO/Hospice Director to build/expand SCAH Home Health to Hospice ‘Bridge’ Program.

Work closely with CMO to prioritize and maximize physician home visit program, particularly as relates to medically necessary visits.

Support and update after hours hospice physician call schedule.

Work closely with Hospice Quality Manager in regards to physician and nurse practitioner documentation audits, quality goals, bioethics consults.

Work closely with Hospice Education Manager to support education within SCAH, Sutter Health- including GME program lectures and precepting opportunities.

Develop and deliver annual curriculum for SCAH Hospice physicians

Observe IDG in each location at a regular cadence for compliance and opportunities of improvement.

Responsible for the medical leadership, management and provision of services. Assures that clinical services delivered are compliant with all laws and regulations.

Job Description:

EDUCATION:

  • Doctorate

TYPICAL EXPERIENCE:

  • 6 years recent relevant experience

CERTIFICATION & LICENSURE:

  • MD-Doctor of Medicine M.D. or D.O. Degree with Post Graduate training in Occupational Medicine

Preferred CERTIFICATION & LICENSURE:

  • Hospice Medical Director Certification
  • Board Certification in Internal Medicine and/or Medical Subspecialty
  • At least 5 years of Hospice Experience (including Management of physicians)
  • Current unrestricted California and DEA licensure

SKILLS AND KNOWLEDGE:

  • Maintains a valid license to practice medicine

  • Works collaboratively with other medical directors. Coordinates issues which may be unclear with regard to responsibility

  • Maintains the highest standards of professionalism consistent with established policies and procedures

  • Accepts responsibility for all properly assigned tasks.

  • Knowledge of treatment and prevention of disease.

  • Good rapport, able to build trust with patients
