The Telemetry/Neurology Unit at Tradition Hospital specializes in the monitoring of various neurological conditions and diagnoses. Many of these patients are post-stroke patients who have been downgraded from the PCU or ICU. As a Team Center Coordinator on the team, you will provide a high level of customer service to patients, families, visitors and caregivers to promote and support the highest level of patient experience. In this role, you will perform administrative and clerical duties to ensure efficient operational flow while also providing or assisting with basic direct patient care activities under the supervision of a Registered Nurse (RN). A caregiver in this position works days from 7:00AM – 7:30PM.  

 

A caregiver who excels in this role will:  

  • Conduct an in-person greeting, welcome and orientation to the unit for newly admitted and transferred patients. 

  • Respond to patient call lights and make rounds.  

  • Communicate and facilitate communication with patients, families, other departments and health care team members regarding patient care. 

  • Assist with processing patient admissions, transfers and discharges. 

  • Assist patients and families with wayfinding and direct them to available resources. 

  • Assemble and maintain patient medical records and other documents related to patient care. 

  • Assist with unit clerical functions and in trouble-shooting equipment failures, such as call system, portable phones, WOWs and other unit devices.
