Leads Inpatient Hospice interdisciplinary team to deliver high quality, low cost, patient centered care. Patient safety and patient self determination are centric to the mission driven teams in continuing the healing ministry of Jesus with an emphasis on under served and poor, with the aim of extending the ministry into the future by creating financial sustainability. Leads the team to achieve the right care at the right time and right place for patients and families. Inspires Staff to change based on best practices, new standards and new regs. Innovates continually to escalate the quality, patient experience, financial status, growth and to position the organization for health care future direction
Minimum Education
Required Minimum Education: 4 year Bachelor Degree
Specialty/Major: Physical Therapy, Occupational Therapy, Speech Therapy or RN degree
Preferred Education: Graduate Degree(Masters),
Specialty/Major: Physical Therapy, Occupational Therapy, Speech Therapy or RN degree
Combination of post-secondary education and experience will be considered in lieu of degree
Licensure/Certifications
Required: KY license in profession, BLS
Preferred: Certified Case Management (CCM)
Minimum Qualifications
Minimum Years and Type of Experience: 10 years of experience in therapy or nursing
