Position Summary
Schedule referrals and admissions for patients, receive and process orders for referred patients, communicate schedule as needed to those that need it, and provide other administrative support to the referral/admission and patient care team in order to provide quality patient care.
Key Responsibilities
- Receives referral/admit intake phone calls.
- Sets up referrals with family members, scheduling visit for admission nurse, communicating visit with referral source and admission nurse.
- Communicates set-up referrals with patient care managers for resource assignments.
- Prints and faxes requested paperwork to facilities for respite stays and NH replacements. Obtains attending MD signature when required on above papers.
- Faxes scripts to pharmacies and other facilities as appropriate and monitors successful transmission.
- Informs regional offices of incoming referrals/admits for them to set up times.
- Ensures proper and timely data entry for all incoming referrals.
- Runs report and follow up on any pending referrals to ensure timely admission.
- Orders DME for new admissions
- Assists call center manager with other miscellaneous projects.
- Completes verbal orders as necessary during the referral process.
- Demonstrates proficiency in the Suncoast electronic medical record (EMR.)
- Completes and locks documentation on the same day as the contact/communication occurs as per the Medical Records Documentation policy.

Pay Range: Starting at $33.66/hour (based on experience)

Qualifications
- Superior clinical skills
- Excellent written and verbal communication skills
- Strong customer service skills
- Excellent computer skills including working knowledge of Word and Excel
- Data entry and medical terminology knowledge
Certifications, Licenses, Registrations Required
- Current state RN/LPN licensure in North Dakota and Minnesota
