Physical Therapist Responsibilities:

• Conduct assessments of referred patients.
• Deliver direct patient care to evaluate medical condition, functional capabilities, limitations, and potential for rehabilitation.
• Develop and implement treatment programs with tailored goals, guided by the patient's capacity and under physician direction.
• Set realistic patient goals, maintain comprehensive documentation, and educate patients and families on care plans.
• Collaborate with all disciplines for optimal patient-centered treatment and care.

Requirements

• Master’s Degree in the field of Physical Therapy or equivalent
• Current and unencumbered Physical Therapy license for the State of NY.
• Experience in long-term care and/or geriatric care preferred.
• Proficiency in EMR documentation (specifically Wep PT).
• CPR certification (Preferred).
• Bilingual in English/Spanish (Preferred)
