Your Specific Responsibilities:

This position functions as screening coordinator and DOH lead in the School Health Program. This position requires leading, educating and working with all school health screening team members and partners to ensure the delivery of vision, hearing, scoliosis and growth and development screenings along with follow-up services to students in the mandated grades for Collier County Public and Non-Public Schools. The incumbent must be able to work independently and is responsible for directing school health staff, including volunteers and other DOH staff assisting with school screenings, and for the planning /set-up of the school screening environment the day of the screening. When necessary, this position participates in the transport of equipment. This position will be responsible for the over sight of all staff/volunteers at screenings. This individual will function as “Record Custodian” for the school health program data. The position is responsible for ensuring the team completes all data entry and reports required for program each year. This position is responsible for submitting screening services data for entry into the HMS system for services performed by School health staff and will help manage the organization of this documentation in conjunction with the CHN. Data entry into specified data management systems (currently Health Office anywhere and FOCUS) as well as tracking the progress and updating the CHN on staffing needs to meet deadlines will be the responsibility of the incumbent. The incumbent will also work as a liaison with all community partners involved with school health services.  This position will function as a team member to provide education to the public and volunteers regarding school health services when required.  This position assists with updating and revising all School Health Forms and assists with generating all School Health reports, documents, and calendars for screenings. In the event the CHN is not available, this position will be responsible for ensuring the continuity of service delivery. This position is at risk for exposure to blood and airborne pathogens.  Annual TB and Immunization screening is required upon hire. This position assists with Kid Care outreach programs when necessary. Continuously demonstrates the Department’s ICARE values.  This position reports directly to the Assistant Community Health Nursing Director.

Functions as a lead screening coordinator and DOH lead at assigned schools. Functions as the liaison between DOH – Collier staff and volunteers, and the Collier County Public School staff to accomplish required screening services for vision, hearing, growth and development and scoliosis. Assures collaboration with the Public and Non-Public School Screening liaisons in each assigned school so screening area preparation and acquisition of support resources are obtained and so that completion of data/documentation elements necessary for student health screenings can be accomplished. Coordinates DOH-Collier and volunteer staff services for the screening day. Participates in the transport and set-up of screening equipment/ environment.

In conjunction with Assistant Community Health Nursing Director, functions as a lead team member responsible for compilation of information necessary for service delivery data, tabulation, and report generation. Assures that methods of acquiring data sets needed to meet the requirements of school health reporting and monitoring in the Health Management System (HMS) are met. This team member functions as a lead records custodian for the school health program. Incumbent will be responsible for preparation of documentation needed to conduct screenings, the processing of screening data post screening events, and keeping track of follow up documentation and data tracking for the program throughout the screening season and current school year.
