Where You'll Shine:
As an RN with ComForCare, you’ll play a vital role in delivering high-quality, personalized care while supporting positive outcomes for our clients and their families. 
Working within your state’s Nurse Practice Act you will:
Provide services in accordance with the care plan Make the initial evaluation visit and regularly reevaluate the patient’s needsInitiate the plan of care and necessary revisions Coordinate services Inform staff of changes in the condition and needs of the clientPrepare clinical and progress notes as well as summaries for each patient visit Understands and adheres to established ComForCare policies and proceduresTake urgent calls from caregivers, clients and client families when necessary Assist with medication management Monitor and document vital signs and health statusEducate clients and families on care plansAccurately document care and observationsServe as the bridge between non-medical support and skilled nursing care

You’ll Thrive Here If You Are/Have…
A Graduate of an approved school of professional nursingAn active, in-good-standing RN license in the state of practiceTwo (2) to Five (5) years of nursing experienceTwo (2) to Five (5) years of case management experienceTwo (2) to Five (5) years of RN administrative experienceA genuine passion for working with seniorsA flexible, positive, and energetic attitudePrior RN experience in Assisted Living or Memory Care (preferred)A professional image in both appearance and behaviorStrong written and verbal communication skills
