·       A Brief Overview
Coordinates and manages the care of patients in complex or high-risk situations. Ensures that patients receive appropriate and coordinated healthcare services, advocating for their needs, and promoting optimal health outcomes.

What you will do

·       Advocates for patients, ensuring their rights, preferences, and needs are respected and addressed.

·       Develops disease management strategies, educates patients about their conditions, and provides self-care management guidance.

·       Examines medical records and assesses the medical necessity of procedures or treatments.

·       Administers appropriate discharge plans and coordinates follow-up care and support for patients and their families during the transition process.

·       Applies in-depth knowledge of healthcare laws and regulations and ensures that all case management practices align with these requirements.

·       Designs educational sessions on disease management, self-care strategies, and healthcare resources.

·       Provides education and support to patients and empowers them to actively participate in their healthcare decisions.

·       Mentors junior staff, participates in committees, completes training, and develops skills.

·       Identifies opportunities for effective communication among team members, shares relevant information, and promotes inter-departmental collaboration.

For this role you will need Minimum Requirements

·       Less than 5 years work experience

·       Registered Nurse (RN) certification required.

·       Working knowledge of problem solving and decision making skills.

·       Working knowledge of medical terminology.

·       Working knowledge of digital literacy skills.

·       Ability to deal tactfully with customers and community.

·       Ability to handle sensitive information ethically and responsibly.

·       Ability to consider the relative costs and benefits of potential actions to choose the most appropriate option.

·       Ability to function in clinical setting with diverse cultural dynamics of clinical staff and patients.

Education
 

·       BSN, ADN, or RN Diploma required
