About the role
<h2>Overview</h2>
<p>The <strong>Nurse Practitioner Palliative CPOM </strong>facilitates palliative care for Gentiva Palliative Care patients, ensuring a comprehensive, individualized treatment plan, while demonstrating excellent clinical skills and ethical, relevant judgment. Provides diagnosis, treatment, consultation, and follow-up under the direction of the Palliative Medical Director (if required under State law).</p>
<h2>About You</h2>
<p><strong>Specialized Knowledge and Skills:</strong></p>
<ul>
<li><p>Strong leadership and interpersonal communication skills.</p></li>
<li><p>Independent decision-making ability and self-motivation.</p></li>
<li><p>In-depth knowledge of palliative medicine principles, Medicare/Medicaid regulations, value-based care, and interdisciplinary processes.</p></li>
<li><p>Meets continuing education requirements set by Gentiva and licensing boards.</p></li>
<li><p>Proficiency with electronic medical record (EMR) software.</p></li>
<li><p>Willingness to travel 10-20% for training and education.</p></li>
</ul>
<p><strong>Education and Experience:</strong></p>
<ul>
<li><p>Master of Science in Nursing (MSN) or equivalent from an accredited institution.</p></li>
<li><p>Minimum two years of Nurse Practitioner clinical experience.</p></li>
<li><p>Specialty in Family or Adult Medicine preferred.</p></li>
<li><p>Experience with value-based care models and Part B billing preferred.</p></li>
<li><p>Ability to work autonomously in an advanced practice role.</p></li>
</ul>
<p><strong>Licenses, Certifications, and Agreements:</strong></p>
<ul>
<li><p>Current RN license or equivalent as required by state law.</p></li>
<li>Active and Unrestricted Nurse Practitioner certification/license without board actions</li>
<li><p>Collaborative agreement with supervising physician from the Professional Services Corporation.</p></li>
<li><p>Part B Provider certification.</p></li>
<li><p>Federal DEA license; state DEA license if applicable.</p></li>
<li><p>ACHPN certification preferred.</p></li>
<li><p>Valid driver’s license and current automobile insurance.</p></li>
</ul>
<p><strong>Technical Requirements:</strong></p>
<ul>
<li><p>Fluent competence in computer and smartphone navigation.</p></li>
<li><p>Working knowledge of Microsoft Outlook, Word, Excel, and PowerPoint.</p></li>
<li><p>Reliable transportation required.</p></li>
</ul>
