Description

EDUCATION AND EXPERIENCE:

Any combination equivalent to education and experience that could likely provide the required knowledge, skill, and ability will qualify. These would typically be:

  • Bachelor’s Degree required
  • Prefer two (2) years of related experience in a health care setting, working directly with individuals
  • Prefer (2-3) years experience in Case Management
  • Bilingual - English and Spansih


REQUIREMENTS:

  • Must be able to work full-time
  • Must possess or be eligible to obtain a current Class C California Driver License
  • Must have a valid Class B CDL with a clean driving record or be able to obtain a Class B license within 60 day period of hire
  • Must be able to pass all pre-employment screening (background check, drug tests, and references)
  • Upon an offer of employment applicant may be required to submit a copy of their Department of Motor Vehicles Driver’s Record


RESPONSIBILITIES:

  • Refers and assists clients with Medicare and Medi-cal applications, Drug Programs and Co-Pay assistance Programs
  • Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs (e.g., recreational transportation, counseling groups, etc.)
  • Determines if the Foundation’s financial programs are appropriate for clients and assists them with applications for such programs (i.e. Transportation, Josh Farler, KCCF-FAP)
  • Communicates with social workers/financial liaisons at referring facilities regarding applicants/clients and reviews applications with the Patient Services Manager and or Executive Director and or committee
  • Manage a caseload and follow up with applicants before and after committee determination and communicates committee determination and requirements (i.e. Memorandum of Understanding, Liability paperwork, etc.)
  • Requests necessary items for patient services (i.e. gas cards, hotel stay, meal cards, checks, etc.)
  • Works with staff in the disbursement of financial aid.
  • Plans, coordinates and executes the Foundation’s educational programs.
  • Works with volunteers and staff to keep thorough records of all applicants, committee determinations, payments, communications, etc.
  • Attends conferences, trainings, and collaborative meetings provided by outside resources
  • Assess and recommend patients appropriate for public relations efforts
  • Understands and complies with Information Security and Health Insurance Portability and Accountability Act (HIPPA) policies and procedures at all times; uses, protects, and discloses patients’ protected health information (PHI) only in accordance with HIPPA standards
  • Required to drive for long periods of time to outlining areas in Kern County, occasionally in adverse weather conditions
  • Assist with fundraising events, community outreach, and may perform other job related duties as required by their Supervisor (some evenings and weekends are required).

Skills in:

  • Must be able to function independently and have flexibility, personal integrity, and the ability to work effectively with patients, staff, and support agencies.
  • Critical Thinking and excellent verbal and written communication skills
  • Proficient in using computers and computer systems including Microsoft Word, Power Point, Excel, Google and other programs.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility. Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must be able to enter and retrieve data using computer systems such as OncoEMR, systems applications, and other office equipment.
  • Must be able to work with volunteers and staff to execute program requirements
  • Ability to meet deadlines and multitask


Ability To:

  • Assists patients and families with financial questions and makes appropriate referrals.
  • Ability to read, analyze, and interpret periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers, patients, vendors, and the general public.
