Job Summary

Join our multidisciplinary transplant team as a Transplant Social Worker, where you’ll play a vital role in supporting patients and families throughout the transplant journey. This position involves conducting psychosocial evaluations and diagnoses for transplant recipients, both pre- and post-transplant. You’ll develop treatment goals, provide solution-focused counseling, and empower patients to overcome barriers to care. Acting as a patient advocate, you’ll coordinate referrals to community resources and ensure continuity of care.

Key Responsibilities

  • Conduct psychosocial evaluations and diagnoses for transplant patients and their families
  • Develop individualized treatment goals and care plans
  • Provide brief, solution-focused counseling as needed
  • Advocate for patients and facilitate access to community resources
  • Collaborate with the transplant multidisciplinary team to support patient outcomes
  • Ensure psychosocial needs are addressed throughout the transplant process
  • Maintain accurate and timely documentation
  • Participate in on-call coverage as needed
  • Maintain a safe and ethical working environment in accordance with NASW standards

Qualifications

Education

  • Required: Master’s Degree in Social Work or related field from an accredited institution

Licensure

  • Required: Limited Licensed Master’s Social Worker (LLMSW) actively pursuing LMSW licensure in Michigan
  • Preferred: Licensed Master’s Social Worker (LMSW) with clinical designation in Michigan

Experience

  • Required: Minimum 1 year of social work experience in a healthcare setting
  • Preferred: 2+ years of experience in healthcare, with emphasis on community health, case management, and patient advocacy
