Per Diem Field Registered Nurse Supervisor - Suffolk County, LI

We are NOW HIRING for Registered Nurses for Start-of-Care Field Visits and follow up visits in Suffolk County, Long Island.

The RN would assess patients' needs and develop the Plan of Care and complete the follow up visits if required for an RN. If visits are for PT, OT, SLP you will not be required to follow up.

Qualifications:

• 2 years of bedside RN experience
• Homecare experience preferred
• AlayaCare experience preferred
• Start of Care experience preferred
   

Requirements:

• NYS Registered Nursing License and Registration
• BLS
• Physical - within a year
• QuantiFERON - within a year
• MMR Titers
• NPI #
• Malpractice insurance
• Two forms for government ID’s
• Two letters of recommendation
