Assistant Director of High-Risk Medical Planning

SUMMARY:

The Assistant Director as a Licensed Practical Nurse (LPN) manages four (4) well-being coordinators in support of the Community Umbrella Agency’s (CUA) capacity to address the medical and dental needs of the children being served. This department is responsible for managing the care of all children experiencing medical complexities, general medical conditions that make them vulnerable or surgical procedures that require general assessments related to the specific conditions, general therapies and interventions. This department is also responsible for educating the agency and community at large as it relates to community health issues and interventions. This department is responsible for the collection, confidential protection and filing of all medical records and serves as liaison to the DHS medical units.

ESSENTIAL POSITION RESPONSIBILITIES: (and other duties as assigned)

• Work closely with Case Management and Community Engagement in efforts to ensure that tactful planning is occurring as it relates to community health & prevention workshops, trainings and cafes.
• Contribute to the development of systems and strategies for medical record tracking, case-planning, data management and record keeping.
• Work with Quality Assurance Department to maintain and audit current system of evaluation.

• Support the collection, maintenance and synthesizing of information from a child’s medical records.

• Auditing email correspondence between WBC and DHS medical units
• Maintaining demographics for HRMP medical and Specialty trackers
• Manage well-being staff uploads to client/families electronic record including BHIF completion
• Support MPS with overflow of cases received active or monitoring level HRMP interventions.
• Update and send Priority Breakdowns to case management teams as scheduled
• Ensure that all direct reports receive a minimum of 10 hours of child welfare training every year.
• Ensure that trainings around medical complexities are made available to case management teams.

MINIMUM QUALIFICATIONS:

• Active/unencumbered LPN License in the State of Pennsylvania.
• Certificate of completion of an LPN program and preferred knowledge of child development
• Basic knowledge of Microsoft Office, Microsoft Excel required.
• Computer competence and proficiency required.
• Valid Driver’s License and ability to drive agency vehicles.
• Current CPR/First Aid/AED certification
• Current PA Child Abuse Clearance
• Current FBI Clearance
• Current PA Criminal Clearance
