Adventist HealthCare seeks to hire a Intake Clinical Liaison (RN/PT/OT/SLP) who will embrace our mission to extend God’s care through the ministry of physical, mental and spiritual healing.
As a Intake Clinical Liaison (RN/PT/OT/SLP) you will:
• Establishes and fosters strong collaboration with ongoing and potential referral sources; partners with external case managers to provide seamless transition to Post-Acute Services
• Screening of potential patients through electronic medical record review or telephonic, in collaboration with physicians, social workers, and case managers
• Understands rehabilitation, home care, and other post-acute diagnoses and clinical indicators to make appropriate decisions for admission; maintains working knowledge of insurance authorization and benefit requirements.
• Determination of patient’s medical stability and readiness for program participation
• Expedites flow of admissions through collaboration with the Patient Access Clinical Coordinator, physicians, pharmacy and nursing
• Identifies appropriate attending physician for home care patients and obtains physician orders
• Follows re-hospitalized patients for safe resumption of care
• Contributes to the coordination of safe transition to appropriate post-acute setting
Qualifications include:
• Bachelor’s degree in Nursing, Physical Therapy, or Occupational Therapy or related area required
• 3-5 years clinical experience in hospital or post-acute setting.
• Preferred experience as a Clinical Liaison or Case Manager
• Current Maryland license (RN/PT/OT/SLP)
Work Schedule:
Variable part time - as needed
Pay Range:
$34.58 - $51.87If the salary range is not displayed or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.
