Description
Purpose
To perform non-professional direct resident care duties under the supervision of nursing personnel; and to assist in maintaining a positive physical, social, and psychological environment for residents in the Skilled Nursing Facility. To perform these duties in accordance with applicable current federal, state, and local regulations and standards; and to perform these duties in accordance with the mission of Elm Terrace Gardens to promote the health and well-being of individual residents based on their functional abilities and potential.
Shifts available: 12 hours 7a-7p
8 hours 7a-3p, 3p-11p
Resident & Nursing Care Functions - Duties
1 Assist residents with daily washing, dressing, grooming, oral care, bowel and bladder functions, preparation for medical tests and examinations.
2 Assist residents with ambulation and transport, transfer, standing, pivoting, and general mobility needs.
3 Assist residents with positioning into and out of beds, chairs, showers, bathtubs, and scales.
4 Provide nursing functions as directed by the Charge Nurse, including daily perineal care, catheter care,
repositioning of resident, and sponge baths.
5 Measuring and recording of temperature, pulse, and respiration.
6 Weighing and measuring residents
7 Performing restorative procedures and assisting with rehabilitative procedures.
8 Prepare residents for meals and snacks.
9 Identify food arrangement and components of different types of diets.
10 Assist in feeding residents as needed.
11 Perform after-meal care.
12 Assist in the coordination and conduct of activities and therapies as directed.
13 Encourage assigned residents to function according to their capabilities and to participate in the life of
the community.
14 Observe and report changes in resident conditions to the Charge Nurse, including changes in levels of function, appetite, alertness, and behavior, and skin breakdowns.
15 Maintain resident bedside area, personal possessions, and clothing in an orderly manner.
16 Clean resident equipment as directed.
17 Ensure that soiled linens and resident personal clothing are processed according to established
procedures.
18 Operate resident assistive devices, mechanical lifts, geriatric chairs, wheelchairs, and other similar
devices and equipment according to safe procedures and training.
19 Present positive endorsement of Elm Terrace Gardens, its mission, programs, policies, procedures, and
administration to Elm Terrace Gardens residents, staff, visitors, and other constituencies at all times.
20 Report to work as scheduled.
21 Assist with resident transport to and from activities.
Resident Rights Functions - Duties
I Maintains resident confidentiality at all times.
2 Treats residents with kindness, dignity, patience, and respect.
3 Knows and complies with the rules of the residents' rights.
4 Adheres to residents' personal property rights at all times.
5 Promptly report to the Charge Nurse or Director of Nursing all resident complaints and grievances.
6 Follow established policies and procedures for reporting and investigating all resident incidents and allegations of abuse.
Administrative Functions - Duties
l Know and comply with the facility's infection control policies and procedures, including the blood-borne pathogen standard, infectious waste disposal, and such other standards as may be promulgated from time to time.
2 Review care plans daily on assigned residents.
3 Make required entries on nurse aide flow sheets.
4 Record food and fluid intake as necessary.
5 Follow established safety rules and regulations at all times, including fire safety procedures and disaster control.
6 Promptly report accidents, incidents, and unsafe, malfunctioning, and hazardous equipment or conditions.
Administrative Functions - Physical & Sensory Req
l Has the ability to make appropriate written entries.
2 Has the ability to follow care plan instructions.
Other Job Functions - Duties
1 Attend and participate in orientation, staff meetings, training, in-service sessions, and other activities as directed by the Director of Nursing.
2 Must obtain at least twelve (12) hours of documented in-service attendance within a twelve-month period and meet such other training requirements as established from time to time by the Commonwealth's Department of Health, Division of Long-Term Care.
