Position Title: Team Lead, Care Coordination
Department: Coordinated Care
Full Time/Part Time, Exempt/Nonexempt: Full Time, Exempt
Reports To: Supervisor, Care Coordination
Job Summary:
A Supervisor of Care Coordination provides oversight of the day-to-day behavioral health clinical operations department for the Empower PASSE program, which provides Care Coordination services to all members.
The Team Lead will be responsible for assisting and coordinating the activities of a team of Care Coordinators for the Empower PASSE program to ensure the delivery of high-quality and efficient managed care services to members. This position will require a strong background in healthcare administration, exceptional leadership skills, and the ability to collaborate effectively with various stakeholders.
Primary Duties/Responsibilities:
May include, but are not limited to:
1 . Team Management:
- Act as a mentor Provide leadership, direction, and guidance to the managed healthcare team members.
- Foster a positive and collaborative team environment, promoting open communication and professional development.
- Conduct regular team meetings to review performance, address challenges, and share best practices.
- Support team members in achieving individual and team objectives. Serve as a mentor and coach to team members, offering support and guidance in their career development and personal growth.
- Collaborate with other team leads and management to foster a cohesive and high-performing organization.
2. Performance Management:
- Identify individual strengths and areas for improvement within the team. Collaborate with team supervisor to set clear performance goals and expectations.
- Monitor team performance metrics, identify areas for improvement, and implement corrective actions as needed.
- Recognize and reward exceptional performance, while addressing performance issues promptly.
3. Process Improvement:
- Continuously evaluate existing workflows and processes to identify opportunities for enhancement and efficiency.
- Work with team members to identify areas of improvement and implement strategies to overcome challenges.
- Continuously assess and optimize team processes to enhance efficiency and productivity.
4. Compliance and Quality Assurance:
- Ensure that the team is consistently adhering to all relevant regulatory guidelines and compliance standards.
- Oversee the implementation of quality assurance programs to maintain the highest level of service quality.
- Monitor and evaluate the effectiveness of care management activities to drive continuous improvement.
5. Training and Development:
- Identify training needs for team members and facilitate the provision of necessary education and resources.
- Promote a culture of learning and professional growth within the team.
- Stay up to date with industry trends and best practices in managed healthcare.
Qualifications
Education and Experience:
- High School diploma and One (1) year experience in the mental health field.
- Bachelor's degree in a relevant field such as healthcare, social work, Psychology or social services and other healthcare related field preferred
- Knowledge of managed care principles, processes, and regulations.
