About the Role:

As an LPN Licensed Practical Nurse at Spring Creek Healthcare Center, you will be responsible for providing high-quality, patient-centered care to our residents. Your weekend shifts will be a critical time for our team, ensuring seamless continuity of care and attention to our residents' unique needs. Your responsibilities will include:

• Providing direct patient care, including medication administration, wound care, and vital sign monitoring

• Developing and implementing individualized care plans in collaboration with the interdisciplinary care team

• Assisting with activities of daily living, such as bathing, dressing, and grooming

• Maintaining accurate and detailed records of patient care and services

• Collaborating with other healthcare professionals to ensure effective communication and coordination of care
