This position contributes to the successful achievement of the organization’s mission and strategic initiatives by providing leadership, oversight, evaluation, and direction of program supervisors to assure tasks are completed & schedules are maintained. Assures programmatic requirements are met based on various oversight/funding entities: DBHDD, CMS, HUD, DCA. Provides budget oversight & monitoring and development & monitoring of programmatic & financial outcomes reports.  Ensures that staff are appropriately trained and current in training.

Provides a broad range of paraprofessional social services to consumers and their families. Participates in treatment Team planning and performs crisis intervention. Assists in Dealing with personal and social problems. May provide Supportive counseling to clients and families and/or serve as a liaison for social services.

Job Responsibilities and Performance Standards:


Supervises and plans work of assigned staff.


Ensures program compliance with all federal, state, and local licensing and operating standards.
Develops, implements, and maintains program policies and procedures as needed.
Continuously evaluates service delivery effectiveness in meeting the needs of clients.
Continuously monitors existing operated and contracted sites in accordance with established guidelines.
May procure or develop new sites based upon identified client’s population needs.
Makes site visits to assist in the implementation of programs and monitors the use of resources as necessary.
Thoroughly conducts or delegates the assessment of programs; provides feedback and recommendations based on findings and in accordance with established policies and procedures.


Manages human resource and employee relations functions.

Works cooperatively with appropriate staff to identify potential candidates for placement.
Works with parents/families of clients to assess and determine client needs in a community/family placement setting.
Develops an effective individualized placement plans for clients.
Effectively monitors client behavior and progress for placement suitability.
Assigns a case manager to each program client to ensure services delivered meet identified client needs.
Assesses needs of target population through proficient and thorough use of surveys, literature searches, focus groups and other research methods.
Meets program deadlines for all assessment tasks.


Establishes and maintains relationships with other agencies and organizations in the community to meet community needs and prevent duplication of services.

In accordance with program criteria and established guidelines, identifies and/or interviews potential residential providers for clients.
Secures all needed support services for clients.
Continually assesses each placement for quality and effectiveness of services delivered.
Continuously monitors clients placements for acceptability and if needed, takes steps to remove client from unacceptable placement situations.
Obtains and/or monitors eligibility for SSI and/or other subsidies for clients.


Assists clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services.

Interviews clients and/or families using acceptable techniques appropriate for the individual's level of functioning in accordance with established guidelines.
Contact other health care professionals within or outside the organization to obtain client’s background information in accordance with established guidelines.
Reviews client’s chart to obtain background information necessary to complete an assessment within established timeframe.
Completes and places in client's chart a written individual/family intake profile and updates information according to accepted standards and time frames.
Complies with regulations on confidentiality of client information as specified by established policy.


Assists client living in personal care homes and halfway houses; assists in pre-release and release planning.

Schedules follow-up appointments in a timely manner to ensure services are provided.
Makes appropriate referrals for support services.
Completes or monitors the completion of required forms within prescribed time frame.
Meets with client's continuity of care representative as required.
Transports or makes arrangements for transportation of clients to appointments in a timely manner.


Implements and organizes the delivery of specific social services within the community.
