Contract:          Full-Time & Permanent

What are we looking for?

Cleveland Clinic London are looking to recruit a Clinical Pathway Coordinator to join our Case Management team, with a salary up to £56,678.84 on offer.

This is a Monday – Friday, 9 – 5 role with no overtime or on-call.

The purpose of the clinical coordinator role is to manage the daily operations and processes in relation to a specified group of patients. The post holder will take an active role in coordinating the refinement of new patient pathways as service lines are stood up until they are embedded as business-as-usual processes.

What will your duties include?

  • Manage and oversee all aspects of care pathways for current services, including Alzheimer's drug therapies, headache services, sleep studies and any additional clinical services identified by the organisation.

  • Lead and coordinate multidisciplinary project teams during the introduction of new service lines.

  • Works closely with Clinical Nurse Educators to design, implement, and evaluate training programmes for staff. Facilitates ongoing education to ensure competency and awareness of new or updated pathways and therapies.

  • Works to ensure seamless integration and efficient patient flow across multiple departments.

What we need from you?

  • Solid background in clinical nursing, ideally with experience across a range of hospital or community settings.

  • Familiarity with patient care pathways, particularly in areas such as case management, discharge planning, and the introduction of new therapies.

  • Previous experience acting as a point of contact for patients and families, addressing concerns, and coordinating care throughout the patient journey.
