Job Summary
As a Clinical Manager Assistant LVN, you'll work closely with the Director of Patient Care Services and Clinical Field Staff to ensure smooth operations and optimal utilization of resources. We value teamwork, communication, and continuous learning to enhance our service delivery.
In this crucial role, you'll provide valuable assistance to our clinical operations in a home health setting. Your responsibilities will include coordinating staffing patterns, analyzing staff visit data, facilitating interdisciplinary communication, and staying updated on regulations and developments in the nursing field. Additionally, you'll maintain open communication with physicians, patients, families, and pharmacies, ensuring efficient coordination between departments.
How You'll Spend Your Time in This Role:
- Coordinating staffing patterns to optimize clinical staff utilization
- Reviewing interventions and assigning patients to clinicians
- Analyzing staff visit data to ensure productivity standards are met
- Facilitating interdisciplinary communication and training
- Staying updated on regulations and disseminating relevant information
- Maintaining open communication with healthcare stakeholders
- Creating and maintaining orders to support field clinicians
- Providing follow-up assistance and performing chart audits
- Accurately entering medication information and performing related duties

To succeed in this role, we're looking for candidates who have:
- Completion of an accredited Licensed Vocational Nurse (LVN) program
- Current and active LVN license in the state of practice
- Minimum of four years of clinical experience in a home health care setting or equivalent combination of education and experience
- Strong knowledge of Microsoft Office Systems and computerized systems in general
- Working knowledge of State, Title 22, and Federal Medicare Conditions of Participation regulations
- Familiarity with community health principles and practices
