We are looking for a dynamic LVN with a desire to set an example of exceptional care and integrity. As an LVN with our home health team, you will perform hospice visits in assisted and independent living communities, including the pronouncement of death covering North Austin - Leander, Round Rock, Cedar Park, and Georgetown.

Key Responsibilities:

• Provide compassionate hospice care to patients in assisted and independent living communities.
• Perform pronouncements of death.
• Manage a fast-paced, independent work schedule.
• Utilize Homecare Homebase EMR software (knowledge of this software is a plus).

Requirements

Qualifications:

• Licensed Vocational Nurse (LVN) with a minimum of one year of hospice experience.
• Proven ability to manage a fast-paced, independent work schedule.
• Strong commitment to providing exceptional patient care.
