Once a patient has a care provider
- Multidisciplinary care coordination is crucial to ongoing care engagement and providing patients with proactive supports while being responsive to the variety of clinical needs patients have over the course of treatment.
The Staff Nurse functions at the intersect between non-clinical patient service coordinators, licensed social workers and treating providers
- Serving as the conduit for clinical triage decision making and coordinated care collaboration with the patient and family.
The Staff Nurse works with two primary distinct populations in the Department of Psychiatry
- Patients cared for in the Wang 8 Injection Clinic and those cared for on the Psychiatry Urgent Care Clinic, who are engaged in Psycho-Pharmacotherapy with one of our prescribing providers.
Primary duties:
• Staffing the injection clinic which includes: safely administers medications and escalated concerns or assessments of patient response to the prescribing provider, accurately completes nursing documentation in EPIC according to policy and procedure, understands end-of-visit and follow-up process/procedures, Ensures the clinic is in compliance with hospital and regulatory standards to ensure a safe care environment, Support and educate patients in self-management, care coordination, medication reconciliation, nutrition, and safety
• Implement the individualized plan of care through the use of evidence-based protocols and guidelines and in collaboration with treating providers
• Coordinate with other members of the health team, including treating providers, social work, nutrition, and community resources
• Coordinates involvement of the patient, family and health team members in patient care, including patient/family teaching and care planning.
• Communicates the plan of care and other pertinent information to/from other health team members.
• Helps define standards of excellence for patient care; participates in improvement of patient care services in a cost-effective way.
• Collaborates with other professionals and directs nonprofessional nursing personnel in maintaining recognized standards.
• Teaches and directs all non-nursing personnel for whom s/he is responsible for clinical oversight: medical assistants and supports the administrative staff in triaging medication questions or symptom management concerns.
• Participates in orientation of medical assistant and nursing staff members.
• Interprets hospital and departmental policies and procedures to nurses and other health team members.
• Participates in daily operational activities necessary for safe patient/staff environment.
• Provides input to Group Practice Administrator regarding unit needs.
• Participates in unit and departmental committees for formulation of nursing and hospital policies and procedures.
Remote RN Requirements:
• Coordinating discharge planning
• Review discharge plan: follow up appt, prescribed RX
• Medication management: adherence + potential side effects, monitor taking as prescribed, ability to fill
• Crisis prevention planning: what to do if patient begin to experience worsening symptoms
• MD/NP support for medication
• Medication reconciliation: confirm RX at discharge, ensure continuity of care
• Medication education: questions about side effects, what to do if patient has negative reaction or missed doses
• Medication assistance: difficulty affording/obtaining medication > connecting patient with assistance programs, completing PA’s
• Patient support
• Family/caregiver education: patients with complex conditions, how to recognize worsening symptoms, effectively supporting
• Psychosocial support: check in about stress management, treatment planning, lifestyle changes
• Symptom monitoring: prevent escalation + need for ED
• Risk assessment: risk for self-harm, suicidal ideation
• Preventative measures: clear care plan
• Acute-needs assessment
