Schedule: Full Time – 80 hours/pay period – Monday to Friday – 8 a.m. to 4:30 p.m.
As a Nurse Navigator, you will be a vital link between healthcare facilities, patients, and Vivie’s home health and hospice services. Your role is to ensure seamless care transitions, build strong relationships with key referral sources, and enhance patient access to high-quality care. By providing education, personalized support, and expert guidance, you’ll help drive admissions, increase service utilization, and ensure an exceptional experience for patients and families navigating their healthcare journey.

As a Nurse Navigator, you will:
- Facilitate Seamless Care Transitions – Conduct pre-discharge calls and visits to assess patient needs, ensuring timely and appropriate admissions to home health and hospice services. Obtain consent for services and complete admissions when necessary.
- Develop and Strengthen Referral Partnerships – Build and maintain relationships with key contacts at assigned facilities, educating them on Vivie’s services and benefits to drive referrals and increase service utilization.
- Provide Patient-Centered Support – Offer a "white glove" experience for patients, families, and referral partners, ensuring smooth transitions and a positive customer journey. Address challenges and resolve concerns to enhance client and facility satisfaction.
- Drive Sales & Monitor Transitions – Create and execute strategic sales plans to achieve admission and census goals. Participate in rounding with healthcare teams to identify service needs and opportunities. Manage discharge follow-up calls and hospice readiness transitions, collaborating closely with clinical teams to ensure timely and appropriate patient referrals.
- Enhance Education & Outreach – Develop and implement annual education plans in partnership with marketing, sales, and clinical teams. Attend facility events to promote Vivie’s services and enhance visibility.

This job also requires:
- Current RN license in the state of Minnesota.
- Minimum 2 years of experience in home health, hospice, care coordination, or case management preferred.
- Experience in sales, business development, or healthcare outreach is a plus.
- Ability to work independently while collaborating with internal and external stakeholders.
- Proficiency in using CRM tools and electronic health records to track and manage referrals.
- Ability to travel locally to assigned facilities as needed.
- Ability to pass state mandated background check.
- Physical capability to perform essential job duties and functions.
- Ability to read, write, and speak English to ensure effective communication amongst team, staff, and families.
