Park Nicollet is looking to hire a Home Care/Hospice Supervisor to join our Home Care and Hospice team!  Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.   

Position Summary:

The Supervisor is responsible for supervising, planning, and coordinating the delivery of individualized care and comprehensive service to patients and families. The Supervisor is also responsible for coordinating training, scheduling, and supervision of nurses and aides providing services to patients in the home setting. The Supervisor supports the following major Community Care processes: admission, discharge, referral, delivery of care, complaints, staff orientation, training and development, and billing

Additional Information:

This role is part of the hospice interdisciplinary team. The supervisor has direct involvement in overseeing the plan of care for patients from admission to discharge. They are involved with the oversight, education, and training of nursing personnel and help to guide high quality care using critical thinking skills and hospice best practice standards. 

This position is primarily 8:00 am- 4:30 pm M-F. This is a hybrid position, working both from home and in the office. This position oversees employees at all HealthPartners hospice locations. Onsite oversight may be needed at times and require travel to the sites in addition joint visits with employees in which travel to the patient's home would be required. This supervisor role is part of the leader on-call rotation. 

The ideal candidate would have previous hospice experience as well as prior leadership and supervisor experience. The ability to work within a model team is essential. The ability to be organized, plan tasks autonomously, and develop /maintain interpersonal relationships with peers is important. 

The PN hospice team is warm, kind, and welcoming. Many of the employees have been with the organization for multiple years. The culture is one of support for each other. Staff satisfaction surveys and patient quality surveys are consistently strong. 

This nursing supervisor role will oversee the Registered field nurses in hospice. 

Work Schedule: Primarily 8:00 am- 4:30 pm M-F. On-call rotation.

This is a hybrid position, working both from home and in the office.

Required Qualifications:

  • Education, Experience or Equivalent Combination:
    • Associates degree
  • Licensure/Registration/Certification:
    • RN License in MN 
    • Individuals are responsible for the travel arrangements required to meet the expectations of the position
  • Knowledge, Skills, and Abilities:
    • Must possess knowledge of reimbursement regulations 
    • Demonstrate knowledge of age-related growth and development principles necessary to provide appropriate services and assure safety of patients
    • Proficiency using Personal computer

Preferred Qualifications:

  • Minimum 3 years’ experience in home care / hospice 
  • BSN/PHN 
  • Previous supervisory experience preferred
  • Excellent written and oral communication skills and ability to speak to groups
  • Committed to the philosophy of customer service
  • Ability to problem solve and interface with primary customers
  • Ability to relate to health professionals and to work with and through others
