Job Summary
Facilitates and manages a comprehensive bereavement & grief support program for families who experience the death of a family member. Educates and supports healthcare team members in caring for families of dying family members.
Major Responsibilities:
Coordinates bereavement efforts across facility or designated service line.
Facilitates development/review of bereavement care guidelines that are consistent across various units.
Collaborates with ancillary services (including social work, case management, chaplains, child life, music/art therapy) in care planning for bereaved families.
Collaborates with healthcare team to provide emotional support and resources to patient & family during and after end of life.
Maintains a current directory of community services that are available for bereaved families.
Promotes an inclusive environment for healthcare teams by implementing bereavement training programs and educational offerings.
Provides guidance, support, and resources to teammates to help them address their own grief.
Organizes annual remembrance ceremony for bereaved families.
Leads hospital-wide bereavement committee.
Education Required:
Bachelor’s degree in Social Work, Counseling or related field required.
Experience Required:
Minimum of 2+ years of acute care experience required.
Bereavement experience preferred.
Physical Requirements and Working Conditions:
Frequently works with computers, phones, and MS Office applications.
