Pacific Clinics serves children, transitional-age youth, families, adults, and older adults. We offer a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education and early childhood education programs to Medi-Cal-eligible individuals and families throughout Alameda, Contra Costa, Fresno, Kings, Los Angeles, Madera, Orange, Placer, Riverside, Sacramento, San Bernardino, San Francisco, Santa Clara, Solano, Stanislaus, Stockton, Tulare and Ventura Counties.  

Compensation We Offer  

  • The initial compensation for this position ranges from $31.40 - $38.61 per hour.  
  • Salary is dependent on commensurate experience above the minimum qualifications for the role and internal equity considerations. 
  • The salary may also vary if you reside in a different location than the location posted.   
  • 8% License Differential for LMFT, LCSW, LPCC, PsyD. and select Peer Certifications. * 
  • 7.5% Bilingual Differential for qualified positions* 
    • *Must meet company policy eligibility requirements.

About our Program: First 5: contracted with SCC Mental Health and FIRST 5 to provide services to children 0 to 5 and their families or support system. Services include Home Visitation and Therapeutic Services.

Schedule: Mon-Fri, 9-5 pm 

JOB SUMMARY

Works within the vision, mission, and philosophy of the agency. Provides assessment and related developmental and psychosocial services to youth and their families by engaging and partnering with parents/caregivers so that they can meet the developmental needs of a youth. This position will be part of a multi-disciplinary team.

RESPONSIBILITIES AND DUTIES

  1. Provides direct support to youth and families in a variety of settings.
  2. Conducts assessments of youth.
  3. Participates in multi-disciplinary team to develop initial treatment plans and monitors progress toward goals.
  4. Provides support counseling and crisis intervention for youth and families.
  5. Works with the family in the development and achievement of their case plan goals.
  6. Partners with family to identify and access community based resources and strategies as they pertain to the five protective factors including but not limited to nutritional, medical, dental, drugs and alcohol abuse support services.
  7. Partners with caregivers in the development of agreed upon skills associated with acquisition of concrete needs, positive behavior management and parenting strategies, self care and coping skills, safety planning, and community interactions.
  8. Mentors and supports families to effectively identify and access services to address and resolve emergency situations.
  9. Works with the family to develop reunification strategies and associated safety plans.
  10. Participates in staff meetings, case conferences, training, family meetings, and professional consultation as necessary.
  11. Follows states laws regarding confidential information in verbal and written communication.
  12. Works collaboratively with members of the network partners and their services.
  13. Prepares reports and recording in a timely manner.
  14. Actively participates in team meeting to effectively share issues, opportunities and successes to help ensure the entire multi-disciplinary team is “on the same page” and can perform as a whole at the highest possible level.
  15. Maintains record of all family contact and services used by youth, completes timely and accurate documentation.
  16. Follows up with families after services have been rendered to determine if service was successful or additional service is needed.
