About the role

Primary Function

The Ambulatory Clinical Pharmacist provides ambulatory pharmaceutical care services within the CNEMG offices and Integra ACO. The clinical pharmacist is embedded within primary care offices and provides evidence based, patient-centered medication recommendations to optimize medication regimens as part of the interdisciplinary team. Ensures the safe and effective use of medications while promoting high quality, evidence-based, and cost-effective pharmaceutical care. Provides patient education and facilitates utilization of other resources to maximize patients’ medication therapy success resulting in improved patient outcomes. Serves as the drug information resource for members of the interdisciplinary care team. Provides coordination for patient financial assistance, formulary concerns and works to facilitate care retention.

Responsibilities

  1. Functions as the medication expert on the interdisciplinary care team
  2. Reconciles and assesses medication regimens for adherence and optimal therapy to achieve desired outcomes
  3. Provides evidence-based, patient-centered medication therapy recommendations as part of the interdisciplinary care team
  4. Provides medication and disease state management education to patients
  5. Practices within established collaborative practice agreement(s) consistent with services delineated within the agreement
  6. Participates in the development and/or implementation of quality improvement initiatives.
  7. Collaborates with providers and patients to ensure high quality, cost-effective care.
  8. Documents all encounters within the electronic health record(s) as well as other platforms as necessary. Communicates appropriately with interdisciplinary care team members.
  9. Contributes to achievement of contractual obligations for the ACO including but not limited to comprehensive medication reviews, adherence outreaches, targeted medication conversion initiatives and gaps in care.
  10. Actively participates in Pharmacy Committee
  11. Develops presentations on quality metric data and pertinent clinical topics and delivers to providers
  12. Participates in development of new programs, services and best practices for pharmacy and medical team.
  13. Precepts and actively contributes to the development of pharmacy students
  14. Assists in the development and maintenance of the PGY1 and PGY2 residency program and pharmacy residents.
  15. Actively participates in care retention initiatives.
  16. Assists with precepting pharmacy students.

Job Qualifications and Specifications

Doctor of Pharmacy (PharmD) degree with 2 years of professional experience or Bachelors of Science in Pharmacy with 3 years experience. Current RI Pharmacist license is required. PGY-1 Residency or Board of Pharmaceutical Specialties certification preferred. Medication Therapy Management certificate required within 6 months of hire.
