Hospital/Clinical Liaison – Travel Required
Allaire Health Services is seeking an experienced Hospital/Clinical Liaison to join our growing team in Vermont. This is an excellent opportunity for a motivated healthcare professional with strong relationship-building skills and a passion for driving patient referrals and exceptional care transitions.
Territory Includes:
Central Vermont
Grafton County, New Hampshire
Additional secondary territories as needed
Position Highlights:
Full-time opportunity
Travel throughout the assigned territory is required
Build and maintain relationships with hospitals, case managers, physicians, and healthcare providers
Coordinate referrals and facilitate smooth transitions of care
Represent our skilled nursing and rehabilitation centers with professionalism and integrity
Work closely with facility leadership to support census growth and community outreach
Qualifications:
Experience in healthcare marketing, admissions, case management, nursing, or hospital liaison services preferred
Excellent communication and relationship-building skills
Self-motivated with the ability to manage a travel-based territory
Valid driver’s license and reliable transportation required
