As a Community Support Specialist, you will empower clients to achieve greater independence, stability, and wellbeing by providing community-based behavioral health services. You’ll work collaboratively with clients, families, and multidisciplinary teams to create personalized treatment plans, deliver evidence-based interventions, and connect individuals to essential resources that foster growth and resilience.

Key Responsibilities:
Provide community-based mental health services to children, adolescents, and/or adults including assessment, diagnosis, treatment planning, and intervention implementation.
Collaborate with clients, families, and other stakeholders to develop and implement individualized treatment plans that address the client’s goals and strengths.
Utilize a strengths-based approach incorporating evidence-based practices such as Motivational Interviewing, Trauma-Informed Care, and Cognitive-Behavioral (CBT/DBT) techniques.
Provide behavioral interventions and skills training, including motivational interviewing, interpersonal skill development, and coping strategies.
Coordinate care with other service providers, including physicians, schools, and community organizations.
Maintain accurate and timely documentation of all services in accordance with agency and regulatory standards.
Facilitate PSR (Psychosocial Rehabilitation) group sessions as applicable to client treatment plan goals.
Participate in team meetings, staff development, and ongoing professional training.
Assist clients in accessing community resources that promote long-term independence and stability.
Perform other duties as assigned by leadership.
Knowledge, Skills, and Abilities:
Knowledge of case management, rehabilitation principles, and recovery-oriented practices.
Understanding of mental health and/or substance use disorders and community resource systems.
Strong communication skills with the ability to produce clear, concise documentation and reports.
Ability to work collaboratively, problem-solve effectively, and manage multiple priorities.
Skill in establishing and maintaining professional relationships with clients, families, and community partners.
Demonstrated ability to work independently and as part of a multidisciplinary team.
Education and/or Experience Qualifications:
Bachelor’s degree in Psychology, Social Work, or a related human services field; or
Bachelor’s degree in an unrelated field and two years of related work experience; or
Any four-year combination of higher education and related work experience; or
Associate’s degree in Behavioral Health Support; or
Four years of qualifying experience providing services to individuals with mental health, developmental, or substance use disorders.
