H@H Hybrid Job Description (HVRN/RPM RN)
This is a hybrid position: will have primary responsibility, with secondary training in opposite job position.
HVRN: Responsible for serving as a visiting nurse within the Hospital at Home (H@H) program. Utilizing critical thinking and clinical skills, RNs provide one-on-one acute care and education. Services include infusion therapy, central line care, catheter care/management, wound care, ostomy care/ management and chronic disease management.
RPM: Responsible for serving as a Nurse in the Remote Patient Monitoring (RPM) program. In this role, the RN will support and educate patients with acute and chronic conditions while the patients are at home. The RPM Nurse monitors patients using cutting-edge technology and established disease specific protocols and provides clinical support for patients via telephonic outreach. The Nurse works collaboratively with a multidisciplinary healthcare team to provide the best outcomes for the patient.
This position requires visiting patients in their homes. Employees must complete onsite training and attend meetings in the office in Indianapolis, IN.
IU Health's Hospital at Home program is a new care model that provides acute-level care in a patient’s home as a substitute for traditional hospital care. Additionally, patients enrolled in the program are cared for by a collaborative multidisciplinary team who work together to meet the patients’ goals of care.
This position requires strong critical thinking skills, the ability to multi-task, and effective communication skills. We are looking for a fast learner, with a strong work ethic, and the ability to work autonomously. Strong interests in technology and innovation preferred.

Key Responsibilities
- Collect information from patients to assess their condition
- Make recommendations for further care
- Provide education to patients
- Consult with physicians and other care providers about patient care needs
- Work with medical records technology
- Refer patients to appropriate team members
- Requires an Associates of Nursing (ASN); Bachelors of Nursing (BSN) preferred.
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
- Requires that the RN has graduated from a nationally accredited nursing program.
- Requires Basic Life Support certification through the AHA.
- Other advanced Life Support certifications may be required per unit/department specialty according to patient care policies.
