POSITION SUMMARY
The Behavioral Health Case Manager is responsible for managing an assigned caseload of clients and assessing client needs by developing, implementing, and reviewing service plans, and working with other internal and external resources in meeting/achieving client service needs.

Qualifications
ESSENTIAL SKILLS AND EXPERIENCE:
- High school diploma or GED from an accredited institution is required, Associate’s Degree preferred
- Minimum of one (1) year of experience working in the Behavioral Health field preferred
- Excellent customer service skills
- Excellent computer skills
- Thorough knowledge of current versions of DSM and ICD, preferred
- Ability to complete a Behavioral Health Assessment with appropriate clinical oversight
- Ability to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practices
- Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross
- Bilingual English/Spanish strongly preferred
- Valid Arizona driver’s license
- Valid Level One Fingerprint Clearance Card issued by Arizona Department of Public Safety
POSITION RESPONSIBILITIES/ESSENTIAL:
- Participate as directed in the screening of new service requests within the center ensuring that all individuals are adequately and appropriately served according to their individual needs and in accordance with existing policies and procedures
- Participate in regular interdisciplinary staff meetings
- Accept newly assigned cases in which the diagnostic intake has been completed by the Intake Specialist. Completes case management assessment on all clients entering caseload
- Develop appropriate treatment/service plans with clients as the means for implementing appropriate service and developing effective alliances with clients
