The Health and Social Services Manager will lead a care management team while overseeing day-to-day clinical operations and health plan clinical initiatives to support achieving quality and affordability targets.  The HSS Managers will support a team within Indiana.  Responsibilities will include adherence reporting, audit oversight, process improvement, execution of clinical decision processes, and team management, to include performance management and implementation of program initiatives.

If you reside in Indiana, you will have the flexibility to work remotely* as you take on some tough challenges.

Approximately 25-50% of field-based travel in Indiana.

Primary Responsibilities: 

  • Works with all areas of leadership across the health plan to support and execute medical and clinical program priorities and goals
  • Identifies and implements opportunities to streamline processes, increase innovation, and support growth of medical and clinical operations programs
  • Provides adherence reporting, audit oversight, and participates in audit activities for health plan
  • Leads laterally to ensure auditing outcomes and reporting findings are successfully celebrated and addressed, as appropriate
  • Selects, manages, develops, mentors and supports staff in designated department or region
  • Develops clear goals and objectives for performance management and effectively communicates expectations, and holds the team accountable for results
  • In order to meet the unique needs of our members, have an intimate understanding of the contractual requirements
  • Identify, select, structure, and prioritize process improvement projects, ultimately implementing changes to meet program requirements
  • Conducts monthly audits of staff and participates in all other required audits and business monitoring
  • Ensures standardized execution of workflow processes, including conducting performance audits, quality reviews, and compliance adherence
  • Participates in training and coaching of direct reports as needed
  • Collaborates across Optum and UHG and interacted with Medical Directors, Senior Leaders, Quality, and other stakeholders

Required Qualifications: 

  • Possess one of the following:
    • Indiana licensed registered nurse in good standing with 3+ years of case management experience
    • Indiana Master's Social worker with 3+ years of case management experience
    • Bachelor's and/or Master's degree in social work, gerontology, counseling, psychology, or nursing with a minimum of 2 years full time direct service experience with the elderly or disabled which includes assessment, care plan development, and monitoring
  • 2+ years of experience providing support or care coordination for older adults
  • 2+ years' Leadership Experience with responsibility for team performance OR 3+ years of experience as a UnitedHealthcare care coordinator/case manager with experience serving as a peer mentor and/or coach
  • 2+ years of experience working within the community health setting in a health care role
  • 2+ years of experience facilitating and working in coordination with an interdisciplinary team
  • Intermediate MS Office skills, including Word, Excel, Outlook, and PowerPoint

Preferred Qualifications:

  • Experience with Long Term Service and Supports or Waivers Program
  • Management experience in managed care setting
