Job Overview:
As a Hospice Admission Nurse, you will provide the comprehensive admission assessments for hospice patients. The Hospice Admission Nurse develops the patient's initial plan of care with the patient and/or family. These nurses help establish frequencies for the skilled nurse and aide visits in conjunction with insurance authorizations.
Required: Certifications/Licenses/Experience:
- Active Registered Nurse (RN) in Indiana or other compact licensure state
Preferred Certification/License/Experience:
- BSN
- Six (6) months of inpatient nursing experience
- Six (6) months of home care experience
