The PACE Intake Coordinator is responsible for initial contact with incoming referrals, assisting with engaging and educating the community about the benefits of and services offered by the PACE program, assisting with the enrollment process, and supporting families with obtaining Medicaid. Under the direction of the PACE Center Manager, this role will support the growth of PACE within an assigned service area and works in collaboration with the PACE Program Development Manager. The PACE program's vision is to provide individualized and joyful care through exemplary teamwork serving as many seniors as possible with the best quality-of-life in their communities. PACE offers seniors and their families the care, nutrition, rehabilitation, transportation, and supportive services they need to remain healthy so that they can live in their own home.

WHAT YOU CAN EXPECT

  • Practice Hours: Monday – Friday, 8:00 a.m. – 5:00 p.m.

  • No Weekends, Evenings, or Holidays

  • Communicates an in-depth overview of the PACE program and services offered, either by telephone, electronically, or in-person to interested persons including candidates for enrollment, caregivers, or others in the community interested in the PACE program.

  • Receives and processes referrals; follows up with all potential participants and referrals regarding program enrollment. Manages the work queue in EPIC for referrals. Manages a referral tracking method with up-to-date contact made with potential participants. Enters new referrals into medical records with updated contact notes and convert to enrolled status once enrollment process is complete. Adds new referrals to Epic for tracking purposes then convert to enrolled status once enrollment process is complete.

  • Follows standard procedures to meet timelines within the enrollment process so that work contributes towards census growth targets.  Identifies barriers to enrollment and contributes actively towards continuous quality improvement of the intake department. Contributes to data collection about referral sources, conversion rates, and other methods relevant to the intake process.

  • Works collaboratively with the PACE Interdisciplinary team to support enrollment and decision-making. Collaborates with other departments within the hospital to align interdepartmental functioning, goals, and expectations, collaborates with PACE IDT to schedule all intake and enrollment needs. Collaborates with PACE IDT and Program Development Manager regarding new and existing referrals status. Escalates concerns in a timely and constructive manner.

  • Provides intake services in a manner that advances the Franciscan ministry to meet the needs of the underserved senior population residing within the FSHW service area. Provides additional resources to seniors to address social determinants of health and to support their overall quality of life.

  • Verifies the eligibility of program applicants per eligibility criteria as stated in state and federal regulations. Completes Home Intake Assessments with potential PACE participants and family member(s) and schedules pre-enrollment and/or Center tours. Completes the Level of Care assessments for potential participants wanting to enroll in PACE and serves as a liaison between potential enrollees and government and their third-party partners (Maximus, and FSSA).

  • Provides the participant or family with a resource document checklist for Medicaid, assists with obtaining Medicaid documents, files and tracks the Medicaid application, completes the Medicaid interview, responds to all correspondences received from the Indiana Family and Social Services Administration.
