Associate Behavioral Health Specialist

The WelbeHealth PACE program helps seniors stay in their homes and communities by providing medical care and community-based services. We like to do things a little differently, going the extra mile and making a difference in our participants’ lives.

The Associate Behavioral Health Specialist (ABHS) is accountable for providing behavioral health counseling services under an LCSW level or equivalent clinician in accordance with supervisory requirements from the Licensure Board. The ABHS will work with participants identified by the interdisciplinary team (IDT) to deliver behavioral health assessments, counseling, crisis interventions, and other therapeutic interventions. The ABHS will work independently with clinical supervision and oversight, and will be eligible to accrue clinical hours required for Licensed Clinical Social Worker (LCSW) licensure. The ABHS will collaborate with the PACE physician, social worker, consulting psychiatrist, or psychiatric mental health nurse practitioner (PMHNP), and other IDT members as necessary.

Essential Job Duties:

  • Conduct behavioral health assessments and treatment planning under the guidance of an LCSW level or equivalent clinician, in accordance with supervisory requirements from the Licensure Board, to determine participants' needs, preferences, and goals, and to develop and implement tailored treatment plans
  • Maintain accurate and up-to-date records of counseling sessions, including progress notes, treatment goals, and any changes in participant status under the guidance of a licensed BHS, while following protocols and legal requirements
  • Provide individual counseling using various therapeutic modalities under the supervision of an LCSW level or equivalent clinician, in accordance with supervisory requirements from the Licensure Board, with focus on active listening, problem-solving, and helping participants to set and achieve therapeutic goals
  • Facilitate therapeutic groups in various forms as appropriate to enhance participants' overall well-being under the supervision of an LCSW level or equivalent clinician, in accordance with supervisory requirements from the Licensure Board
  • Provide mental health crisis interventions; recognize signs of crisis situations and provide initial guidance, while escalating to licensed BHS as needed
  • Engage in regular supervision and team meetings with an LCSW level or equivalent clinician, in accordance with supervisory requirements from the Licensure Board, to discuss participant progress, treatment plans, and any challenges encountered in therapy sessions
  • Work under the supervision of an LCSW level or equivalent clinician, in accordance with supervisory requirements from the Licensure Board, receiving regular guidance to ensure professional growth and compliance with clinical best practices; actively participate and engage in clinical supervision contributing to the accrual of clinical hours for LCSW licensure

 

Job Requirements:

  • Master’s degree in social work (MSW) or marriage and family therapy (MFT)
  • Completed state Law and Ethics Exam and issued Associate Clinical Social Worker (ASW) Registration Number in good standing through Board of Behavioral Sciences (BBS)
  • Minimum of two (2) years of experience providing direct services to people with chronic mental illness and substance abuse issues
  • Minimum of two (2) years of experience working with a frail or elderly population
  • Excellent clinical, organizational, and communication skills in settings with frail, elderly participants, their families, and IDT members
  • Ability to facilitate problem-solving with challenging psychosocial and family situations

Salary/Wage base range for this role is $68,640 - $89,535 / year + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits.  Actual pay will be adjusted based on experience and other qualifications.
