About the role

Summary

Under general administrative direction, coordinates referrals from physicians, case management, and health care facilities for patients. Responds to and performs pre-admission screening for patients referred to the rehab program. Provides a significant level of patient education related to illness and rehabilitation recovery process. Responsible for guiding patients with rehabilitation DRGs through the discharge rehabilitation care continuum. Responsible for the development and implementation of referral program as a means to promote the feature and benefits of the rehab program to patients, families and the community. Collaborates among all disciplines to ensure that patient’s receive care that consistently follows evidenced based standards. Completes established competencies for the position within designated introductory period. Other related duties as assigned.

 

Qualifications

MINIMUM EDUCATION: Bachelor’s Degree from accredited college or university in health care clinical field.
 

PREFERRED EDUCATION: Master’s Degree from accredited college or university in nursing 

MINIMUM EXPERIENCE: 

  • 2 years of clinical experience in health care environment, preferably in hospital.
  • Possess strong interpersonal skills and experience working with adult rehab patients and geriatric disabled population.

PREFERRED EXPERIENCE: Marketing experience. Working knowledge of computers and basic software applications used in job functions such as: PC, LAN, Allscripts, E-mail, Microsoft Office, ACCESS, Printers, Fax and other machines.

REQUIRED CERTIFICATIONS/LICENSURE: Possession of current Texas State License in Clinical Specialty

REQUIRED COURSES/ COMPLETIONS (e.g., CPR): Must have active healthcare provider Basic Life Support on hire (ARC or AHA); however, must have AHA Healthcare Provider BLS within 60 days of hire

The RN provides high-quality, safe, cost-effective, total nursing care to all patients. The RN assumes responsibility for overall coordination and integration of patient care based on the nursing process. The RN directs care according to established standards of safety, risk management, QI, and Infection Control, with the goal being to discharge the patient with an optimum level of care provided. Assumes responsibility for coordination and planning of patient care within the prescribed medical regime. Administers all medications safely and in a timely manner per hospital policy. Performs and fulfills all HICS (hospital incident command system) responsibilities, assignments, tasks, and roles as directed and needed by the facility in times of emergency and/or disaster. Responsible for knowing the Hospital’s emergency codes, including the purpose and appropriate action for each, and adheres to the Hospital policies and procedures for each emergency code.     

Education:

Required: Graduated from a state approved school of nursing or an accredited school.
Preferred: Bachelor's or Master's degree in Nursing

Experience:

Preferred: Recent acute care experience.

Certifications:

Required: Currently licensed, certified, or registered to practice profession as required by law, regulation in state of practice or policy; CPR. AHA BLS, and if applicable by corporate policy for unit of hire, AHA ACLS and/or PALS and/or NRP.
