Registered Nurses for Overnight VA Hi Tech Shifts

Activa Home Health (an AccordCare Company) is a Medicare-certified, ACHC-accredited home health agency. We are seeking experienced Registered Nurses (RNs) for night-time hourly hi-tech cases within our VA contract for Martin and St. Lucie County.

These are specialty cases requiring strong clinical skills and the ability to manage complex care in the home setting.

Required Clinical Skills

• Ventilator management (Astral Vent experience required)
• Tracheostomy care
• Tube feeding (PEG)
• IV therapy and medication administration
• Wound care
• Strong assessment and critical thinking skills

Position Responsibilities

• Provide skilled nursing care per individualized Plan of Care
• Safely manage hi-tech equipment and clinical interventions
• Monitor and promptly report patient condition changes to the Nurse Supervisor
• Collaborate effectively with the care team and support family/caregivers
• Maintain accurate and timely documentation in Kinnser/WellSky
• Occasionally complete supervisory visits or assessments

Required Qualifications

• Active RN License – State of Florida
• Current ventilator/trach experience (Astral Vent required)
• Current Experience with high-acuity cases in home health, acute care, or critical care
• Current Level 2 AHCA clearance
• CPR certification
• TB test (within 12 months) or Chest X-Ray (within 5 years)
• Home health and/or VA experience
