Supports Children's Community Health Plan (CCHP) members with chronic health conditions utilizing an integrated, comprehensive approach to care. Assists in the direct and indirect coordination of care through member, caregiver, practitioner and community education while addressing medical, behavioral, educational and social needs.

Responsibilities and Requirements

• Bachelor's degree in Nursing (BSN) required within 1 year of hire

• Registered nurse with a valid, unrestricted Wisconsin nursing license.

• Requires a valid driver's license, acceptable driving record, personal auto liability insurance coverage and the ability to be insured by the CHHS auto insurance carrier.

• Two or more years of nursing experience. Experience in a Medicaid HMO/managed care or public health setting preferred.

• Strong organizational skills.

• Demonstrated ability to prioritize tasks and handle multiple issues while meeting timelines.

• Excellent oral and written communication skills.

• Ability to communicate effectively and work collaboratively with internal departments, external providers and mem
