Case Manager (Part-time)

Are you passionate about helping individuals experiencing homelessness secure permanent housing and supportive services? MHA is seeking a Case Manager to join our dedicated Housing Services team. In this role, you’ll provide outreach, assessment, service coordination, and housing placement support to individuals transitioning out of homelessness.

Schedule: Part-time, 20 hours/week, Mon-Fri 1p-5p

Key Responsibilities:

• Engage participants through outreach, enrollment, and assessment
• Develop person-centered, trauma-informed action plans focused on housing stability
• Connect individuals to vital services such as mental health care, substance use treatment, legal support, and job training
• Provide crisis intervention and develop pre-crisis plans
• Collaborate with community partners, state and local agencies
• Offer transportation assistance and support participants in achieving their goals
• Maintain accurate documentation and meet regulatory requirements

Requirements

• 6+ months of experience working with individuals experiencing homelessness, addiction, or psychiatric symptoms

• Valid driver’s license, reliable vehicle, and auto insurance
• Ability to work independently and as part of a team
• Strong interpersonal, communication, and organizational skills
• Bilingual (Spanish) and/or Bi-Cultural candidates encouraged to apply
