Job Summary
The Care Manager is a Registered Nurse who is responsible for the planning, coordination and supervision of all healthcare plan services for members for an assigned caseload.
Core Job Responsibilities
- Collaborates with member, family, physician, social worker and service providers to insure appropriate member care for mutual achievement of healthcare plan goals and objectives.
- Complies with all federal and state regulations including HIPAA.
- Ability to effectively communicate in English, both verbally and in writing. Additional languages preferred.
- Ability to organize and implement plans, efficient and flexible in problem solving.
- Able to motivate and educate members.
- Proficient computer skills necessary.
Education/Experience Requirements
Required:- Three to four years of professional nursing
- Demonstrates leadership abilities and functions well in interdisciplinary groups.
- 1 - 2 years of case management or homecare experience
Licensure/Certification Requirements
Required:
- RN or BSN.
- Current license from the NY State Board of Registered Nursing.
- Current CPR certification
- Valid state driver’s license and reliable automobile
