Care Manager - Full Time - Days

Job Summary

The Care Manager is a Registered Nurse who is responsible for the planning, coordination and supervision of all healthcare plan services for members for an assigned caseload.

Core Job Responsibilities

• Collaborates with member, family, physician, social worker and service providers to insure appropriate member care for mutual achievement of healthcare plan goals and objectives. 
• Complies with all federal and state regulations including HIPAA.
• Ability to effectively communicate in English, both verbally and in writing.  Additional languages preferred.
• Ability to organize and implement plans, efficient and flexible in problem solving. 
• Able to motivate and educate members. 
• Proficient computer skills necessary.
   

Education/Experience Requirements

Required:

• Three to four years of professional nursing
• Demonstrates leadership abilities and functions well in interdisciplinary groups.

Preferred: 

• 1 - 2 years of case management or homecare experience 

Licensure/Certification Requirements

Required: 

• RN or BSN.  
• Current license from the NY State Board of Registered Nursing.
• Current CPR certification
• Valid state driver’s license and reliable automobile
